Permanent Housing for homeless individuals and families impacted by HIV/AIDS

MIPH provides community-based case management services for up to fifteen HIV/AIDS infected minority residents in Central New Jersey. This Ryan White Case Manager assists clients as they navigate the range of services available for those with HIV/AIDS.

MIPH provides community-based case management services for up to fifteen HIV/AIDS infected minority residents in Central New Jersey. This Ryan White Case Manager assists clients as they navigate the range of services available for those with HIV/AIDS.

These formerly homeless individuals and families include graduates of the Imani Park program and referrals from the New Brunswick Counseling Center, the Raritan Bay Medical Center and the Visiting Nurse Association of Central New Jersey. The case manager works with each program participant to focus on issues of tenancy and housing retention, as well as budgeting and treatment compliance. We coordinate with landlords as needed and make referrals for medical, dental, social service, and mental health and substance abuse services, while helping program participants navigate a range of outstanding legal issues.

This support of a case manager enables the formerly homeless to settle into housing and empowers them as they learn the skills necessary for independent living.

We work with participants to be sure that they:

• remain in case management

• retain housing

• adhere to service plan

• comply with their plan for medical services
